Create/manage patron survey forms

Note: The following information pertains to internal patron survey forms. For information about patron survey options, including external patron survey forms, see Patron surveys.

To create, change, view, or remove internal patron survey forms:

  1. Select Administration in the Select Service drop-down list.
    QuestionPoint displays the Create New Librarian Account page.
  2. Click the Surveys link below the Institution tab.
    QuestionPoint displays the Institution's Survey Report page.
  3. Click the Survey Form tab.
    QuestionPoint displays the Institution's Survey Form page.
  4. If you only want to view the survey forms, go to step 11.
    Otherwise, go to step 5.
  5. Select the Link to the Internal survey defined below button.
  6. Check the Chat Session box if you want to provide a chat survey form.
    Otherwise, make sure the box is not checked.
  7. Check the Ask Answers box if you want to provide an ask survey form.
    Otherwise, make sure the box is not checked.
  8. If the Chat Session box is checked, check the box in the Chat column for each survey question (up to 15) that you want to include in the chat survey form.
    Otherwise, go to step 9.
  9. If the Ask Answers box is checked, check the box in the Ask column for each survey question (up to 15) that you want to include in the ask survey form.
    Otherwise, go to step 10.
  10. Click the Save button.
    QuestionPoint displays an acknowledgement message.
  11. Click the View Internal Chat Survey button.
    In a separate window, QuestionPoint displays one of the following:
    • The chat survey form if the Chat Session box is checked.
    • A message indicating that the survey has been removed if the box is not checked.
  12. Note any changes that you want to make to the chat survey form and close the window.
  13. Click the View Internal Ask Survey button.
    In a separate window, QuestionPoint displays one of the following:
    • The ask survey form if the Ask Answers box is checked.
    • A message indicating that the survey has been removed if the box is not checked.
  14. Note any changes that you want to make to the ask survey form and close the window.
  15. If you want to change the survey forms, repeat steps 5–14.
